So it& #39;s OBVIOUS something is inside of his ear here and it is covering up the inner ear and the ear canal.

This guy looks feeble.
Clear lighting right into the ear, there SHOULD be a little round hole there but there& #39;s not.
Even in LOWER LIGHTING from that angle you should be able to see the ear canal.
