THREAD: For the last year, our ePAC collaborative led out of @UVicNursing has been creating resources & tools to support #ACP conversations among people living & working in the inner city. Now that #COVID19 is here, this work has become even more essential.

The need for #AdvanceCarePlanning emerged long before COVID. It came from the experiences of @OutreachSolid and others in our inner city action team accompanying their clients/peers/loved ones to the hospital when they were no longer able to speak for themselves.
People felt unprepared to deal with the medical/legal questions they were being asked or they weren& #39;t included or listened to as "chosen" family in a medical system that prioritizes biological family. Documenting people& #39;s wishes and family was one way to combat this.
One thing we& #39;ve noticed in our workshops w/ people living in poverty/homeless is that folks are KEEN to do #AdvanceCarePlanning. It has been taken up as a site of self-determination & autonomy in health and social care systems that rarely privilege what matters most to people.
But it takes work understanding how #AdvanceCarePlanning can be incorporated into the conversations that people living & working in the inner city are already having in their everyday so that #ACP adds value and isn& #39;t simply added to the list of competing priorities.
We still have lots to figure out including how #ACP documents are stored and how they are communicated to the health care system. We are slowly working out the systems issues and, luckily, have access to many great minds oriented towards problem solving and innovation!
We have been inspired by @DyingMatters @StMungos and others who have adapted #ACP tools for inner city settings. We are rolling out our ACP process slowly w/ partners to provide adequate support w/ serious illness conversations & get feedback, but look forward to sharing soon!
