I think the most overlooked factors right now are probably:

1) risk of pulmonary microthrombi (practice appears to be converging on prophylactic enoxaparin, see e.g. Yale guidance)

2) secretions, risk of hyaline membrane formation (consider ambroxol or similar, prone position)
